Perth Clinic and Dr Tannenbaum acknowledge the importance of privacy and respect your right to control how your personal information is collected and used. Your information is subject to the Privacy Act 1988 (Cth) (the “Act”). This Privacy Policy (“Privacy Policy”) is aligned with the Australian Privacy Principles as set out in the Act and describes the way that we may collect, hold, and disclose personal information.
This Privacy Policy applies to the services provided by Dr Jeremy Tannenbaum.
In this policy, “Personal Information” means any information that may identify you or by which your identity might be reasonably determined. The information you provide us may include, amongst other things, your name, address, email address, and phone number.
“Sensitive Information” means any information about an individual’s racial or ethnic origin, political opinions, memberships in a political organisation, religious belief or affiliation, philosophical belief, membership in a professional or trade association, membership in a trade union, sexual preference or practices, criminal record, or health information.
“Health Record” means any information we hold concerning your health. It may include your medical records, clinical history, medications, diagnoses, treatment, tests and results, medical procedures, genetic information, file notes, consents, referrals, clinical correspondence, medical imaging, immunisation records, and any other information provided or collected by us relating to your health.
Access & Accuracy

We will respond to your request for Personal Information within a reasonable time. We reserve the right to charge an administration fee to cover the costs of responding to your request, for example, where Personal Information is held in storage.
If required by law or where the information may relate to existing or anticipated legal proceedings, we may deny your request to access your information. We will respond to your request, writing down the reasons for our refusal.
Storage & Security
We prioritise safeguarding your Personal Information and have implemented reasonable measures to protect it from misuse, loss, unauthorised access, or alteration. Our security measures encompass a mix of physical, technical, and administrative controls.
All your Personal Information is stored in our record-keeping system. This information may be securely stored electronically through electronic medical records and third-party data centres. Alternatively, physical storage at our premises or third-party secure storage facilities might be employed.
However, despite our best efforts, it is crucial to understand that no data security measures can always guarantee absolute protection. Transmitting information via the Internet carries inherent risks, and you should be aware of this when sharing information with us.
If we no longer require your Personal Information, we will take appropriate steps to destroy it or permanently de-identify it in accordance with the relevant legal obligations. Your privacy and data protection remain a top priority for us.
Data Breach Notification Scheme
If we have reason to suspect a data breach has occurred, we will undertake an assessment in accordance with the Notifiable Data Breach Scheme. If we determine there has been an eligible data breach, we will notify you as soon as reasonably practicable.
If the breach relates to the My Health Records Act, we may disclose your Personal Information to the My Health Records System Operator under s 73A of that Act.
Identifiers
An identifier is a unique number assigned to an individual to identify them. Identifiers include Medicare Numbers and Tax File Numbers. We will not adopt, as our own, any identifier of you or use or disclose an identifier of you that has been assigned by a government agency unless permitted under the Act.
Collection
We collect personal information to provide you with the best service experience possible on the website and for our internal business purposes that form part of normal business practices. Some provision of Personal Information is optional. However, if you do not provide us with certain types of Personal Information, you may be unable to enjoy the full functionality of our services and/or website.
To provide our services, we may collect Personal Information, such as your contact details, including your name, email address, postal address, telephone number, and business or company name. We may also collect details of conversations we have had with you or any other information relevant to us.
We may collect Sensitive Information about you, such as medical reports, referrals, medication, health history, and other important health information where you consent, and such information is reasonably necessary to provide our services to you.
We automatically collect information that is often not personally identifiable through our Site and services, such as the website from which visitors came to our Site, IP address, browser type, and other information relating to the device through which they access the Site. We may combine this information with the Personal Information we have collected about you.
Use & Disclosure
We will ensure that Personal Information collected is strictly used for its intended purpose, which includes managing your contact details, providing our services, and processing payments. Occasionally, we may disclose your Personal Information to third parties or contractors who play an essential role in delivering relevant services, such as referring doctors and other healthcare practitioners.
Any other use of your Personal Information will align with your reasonable expectations, or the permissions granted by the Privacy Act. Additionally, we may share aggregated data, keeping your identity confidential, for market analysis, research, or marketing purposes to enhance our services and understand health-related matters.
Your Personal Information will never be sold, shared, or traded with third parties unless we have obtained your explicit consent.
In the unlikely event of business-related changes like a sale, merger, or transfer, customer information, including Personal Information, may be transferred to new entities. Similarly, in the rare event of insolvency, Personal Information may be conveyed to relevant parties. However, your objections to such uses of your information will be considered.
When it comes to health information, we may share it with other medical service providers, such as general practitioners and allied health professionals, only with your consent. In certain situations, required by law, like referral to health service providers, billing, liaising with government offices for Medicare matters, or when necessary to protect life, health, or safety, we may also share health information.
Anonymous Health Care
We respect your preference for anonymity when seeking health services. While we make every effort to comply with your request to use our services anonymously or under an alias, there might be situations where it is either unlawful or impractical for us to do so.
Please rest assured that your health care will always remain our top priority. However, in certain cases, providing services anonymously could compromise the effectiveness of your treatment or health outcomes.
For administrative and regulatory reasons, we are unable to provide Medicare rebates or access to Veterans Affairs entitlements without proper identification. If you choose to remain anonymous, you may not be able to access the Pharmaceutical Benefits Scheme in case you require any medication.
Under the Public Health Act, we are legally obligated to report certain medical conditions. If, while providing our services, we diagnose a prescribed medical condition, we must make a report, including your identity, to the Health Department.
Cookies, Web Beacons, and Analytics
We strive to make your experience on our Site easy and meaningful. For this purpose, we, or our third-party service providers, may use cookies, web beacons (clear GIFs, web bugs), and similar technologies to track site visitor activity and collect site data. Any information collected is combined with the Personal Information we have gathered from you and will be handled in accordance with this Privacy Policy.

If you wish to stop receiving these promotional emails, you can follow the opt-out instructions contained in the email. Please note that it may take up to 10 business days for us to process opt-out requests. However, please be aware that even if you opt out of promotional emails, we may still send you emails related to your account, services you have requested, or other customer service purposes. We do not provide your Personal Information to other organisations for direct marketing purposes.

This Privacy Policy may be updated occasionally. Please periodically ask for our current Privacy Policy to stay informed about any changes. By continuing to use our services, you accept and understand the Privacy Policy in effect at the time of your use. If any material changes affect how we handle the Personal Information we have collected from you, we will strive to notify you in advance of such changes by highlighting them, where practical, by email.
